Required Forms:

• Background check and copy of driver's license for each person who will be co-oping in the classroom 
• Health form for all children as required by State law must be submitted each year.
• Notarized authorization for emergency medical treatment for new children. 

No child may attend school without all required immunizations, health form and notarized emergency authorization on file.

No parent or other person may co-op without background check on file.  You may mail forms to school (click here for address) or bring them to Orientation.
